CREDENTIALLED DIABETES EDUCATION

What is a Credentialled Diabetes Educator?


Credentialled Diabetes Educators (CDE’s) are healthcare practitioners qualified to provide a client-centred approach to diabetes education and care, empower patients and assist them in dealing with daily self-management, in accordance with the National Standars of Practice for Diabetes Educators.


Services provided by a CDE.


High quality diabetes self-management education for all types of diabetes across the lifespan and the continuum of the disease progression, from newly diagnosed, through to type 1, type 2 and gestational diabetes review of management, and end of life care. CDEs are equipped to provide self-management education through person-centred care regarding lifestyle modification, appropriate blood glucose monitoring, complications screening and management, psychological screening, monitoring using appropriate pathology, and diabetes medication. CDEs are recognised by NDSS (National Diabetes Services Scheme), Medicare Australia and the Department of Veterans Affairs, as well as Private Health Insurers.
